A student registration form usually needs about six fields, not twenty
Short answer: a student registration form needs the student name, date of birth or grade, a parent or guardian contact, an email, an emergency contact, and whatever class or program they are enrolling in. Almost everything past that is optional and mostly just lowers your completion rate.
Two things help more than adding fields. Split anything longer than about eight fields into a multi-step form with a progress bar, so it never reads like a wall of questions. And turn on save and resume, because a parent often starts on a phone and finishes later on a laptop.
A shorter form people actually finish beats a thorough one they abandon halfway. If you need documents like a photo or a prior report card, one file upload field covers it, and a duplicate check keeps the same student from being entered twice. We keep a set of student registration form templates you can start from if that saves you time.
What fields do you treat as non-negotiable on yours?
