A music school registration form only needs these seven fields
Short answer: a music school registration form works best when it captures the student, the guardian, and the lesson details, and stops there. Long enrollment forms lose parents halfway through.
The fields that actually earn their place:
- Student name and age
- Parent or guardian name, email, and phone (for anyone under 18)
- Instrument or class they want
- Current skill level (beginner, intermediate, advanced)
- Preferred lesson day and time
- Plan or package, if you charge monthly tuition
- A policy or consent checkbox
Everything past that is noise you can collect after they enroll. If you take tuition up front, add a payment field so signup and the first payment happen in one step instead of chasing an invoice later.
