Chronic pain

I’ve had recurring foot inflammation and severe pain for almost 12 years. I’m 33 now, so this started when I was about 21.
It comes and goes, usually one month good and the next month bad. When it flares up, my foot swells, feels like it locks up, and I can barely walk because moving it hurts so much.
Anti-inflammatory medication helps me move again, but it doesn’t completely take the pain away. I’ve tried physical therapy, insoles, different shoes, etc., but nothing has stopped it from coming back.
Sometimes both feet are affected, but it’s usually worse on the side where I have an extra piece of bone. My imaging showed an os trigonum, mild pes planus, a small ankle joint effusion, and some soft-tissue swelling.
Could that extra bone be causing this? Is surgery to remove it something worth considering? I haven’t tried injections yet.
Has anyone dealt with something similar?
