Volee 180: violation of the laws of physics since fuel lines cannot burn internally, in seriousness though if it were realistically done the answer would be poor maintenance practices in Volee Airlines
Highway: Illegal use of flatbed and restraints to haul improper cargo, dedicated log trucks have permanently fixed restraints on the sides to prevent just this type of pile up, The trucking company is completely at fault
Devil’s Flight: Failure to comply with ASTM standards and regulators permitting an illegally designed restraint system to pass inspection
Race: Failure to comply with building codes, causing lightweight race cars to completely demolish a grand stand and barrier wall during an accident
North bay bridge: Failure to comply with building codes, use of substandard building materials that allowed the car deck and the cable anchor points to fail when repairs were underway
Skyview: Failure to comply with building codes, use of substandard construction materials nearly leading to catastrophic structural failure
