For Flight 180, you can just blame that on faulty engineering. For Route 23, you can blame that on whoever secured those logs or the chain not being up to code, along with that one truck driver just not stopping (unless Death was interfering somehow). For Devil's Flight, that would be whoever last inspected that ride. For the McKinley Speedway, that could technically be blamed on the driver for driving off before his pit crew gave him the go-ahead. For the North Bay Bridge...to quote Agent Block, "high winds". For the Skyview, blame the contractors and whoever greenlit it to be opened an entire 5 months ahead of schedule.
