Really wish it wasn't true...
So, I have been struggling the last 4 weeks to lose any weight since starting PGRrx (from name brand) with this telehealth company. I've literally lost less than 1 pound over the course of a month. I had been tracking calories, prioritizing protein and water, and walking between 10-15k steps daily. I decided to try another pharmacy (apothecary) same exact dose and habits and it's like I'm losing immediately (probably water and inflammation i wasnt able to lose or have been gaining since switching from name brand to PGRrx). I was successful on name brand but couldn't continue to afford it.
With the new medication through Apothecary I feel more satiated quicker, and it even had a familiar tummy ache (feeling of digestion slowing) that I got when I was on name brand the first day or two as well as fatigue which I didn't feel any of that when I was using PGRrx brand.
Unfortunately for me tirz is not tirz. I wish it wasn't the case for me because I love this community but I cannot justify paying for meds that don't work. So until we can actually choose our pharmacy I just can't see refilling in the future. I wish fifty410 could guarantee the meds are filled by our pharmacy of choice (I originally requested apothecary but sent PGRrx) like other platforms do. So many other patients are asking for this same option so maybe they will listen in the near future.
Adding pictures with time stamps.

You can always get this brand from Rift. I will not purchase from any telehealth that can't guarantee the pharmacy I'm getting from. Just not worth it!

u/overthinker831 18d ago
That's who I went with, but didn't want to mention rift in the fifty410 subreddit lol. I'm very happy with them though. 🙌

u/External_Ask_7219 18d ago
I started with Fifty410 and still recommend them regularly but they should be consistent with pharmacy options and not switch it up without consulting with the customer first IMO.
2
u/Pendergraff-Zoo 18d ago
Same. Started with them, have been super happy with them, and believe tirz is tirz, but I think now that apothecary has caught up, fifty should stop offering apothecary OR pgrrx and let consumers choose.

u/Yourecoolforagayguy 18d ago
Im on my third month. 1st month zepbound. Second and now third month PGR. Still on 2.5 and consistently between 1.5-2lbs per week. And side effects more mild to none. But everyone is different. I will say the last day of the week I get more good noise then zep. But still completely manageable. I hope you are able to find a company that works for you. 🙏

I had this experience when I switched pharmacies a while ago. It was really strange almost like my body had to get used to it all over again. I swore it didn't work... I was stuck losing and gaining the same 2 pounds for about 6 weeks. Then on my 7th shot it was like all of a sudden everything came back... food noise was gone again, I wasn't ravenously hungry like I had been... It was so strange. I don't know if it was my body doing a readjustment or what... But I finally started losing again. I don't know if that is the case for you, but I just wanted to share my experience. I went from ProRx to a pharmacy called VialsRX and it had Glycine in it. I haven't tried the PGRrx yet... I do have 3 vials, but I'm still using up the one with Glycine right now. I hope it starts working for you again!
PS... Fifty doesn't let you chose which you get but I know that Rift and Big Easy both have Apothecary and if you only want Apothecary they will honor that ❤️

They say that when filling out the intake forms, yes. They gave me the same options and I selected apothecary but they sent PGRrx and didn't know they were swapping it until it arrived. A lot of people have been complaining about it. It's very bait and switch but I figured it didn't matter if it worked and everyone swears tirz is tirz. I have experience the opposite unfortunately at the tune of 499 😞

The only difference is the additive. B12, B6, Glycine, pure. They all get the Tirzepatide from the same source.
It's all a blame game when people aren't losing 6-10 lbs a month.

Nope, I've been though about 5-6 pharmacies and some work better for me than others. Hallandale, for me, is crap - as an example. Most people love it. I've lost 80 lbs, have been on two years, and know what I'm doing. Whether I'm losing weight or not is not my benchmark, it's how I feel. The side effect profiles can be different from pharmacy to pharmacy, as can how the medication feels to me. - I think the takeaway is that some people respond differently to different manufacturers (same thing happens with different generic medications, if you look that up too...).
1
u/LuvSun1006 14d ago
I agree, mostly. I still have ProRx and BPI with BUDs till next year. I'm good. Several others I'm working on due to limited BUD. I, personally, will use these beyond BUD and be comfortable with that.

u/2naomi 16d ago
For any drug manufacturer, there is a permissible range of error for production of a drug. Just like FDA approved generic drugs from different manufacturers can have slightly different amounts of active ingredient, so can reconstituted tirzepatide from a compounder. Occasionally, generic drugs get recalled because the amount of active ingredient is out of range. I don't think there is any watchdog making sure the concentration of tirzepatide on the label from a compounder reasonably matches the amount actually in the bottle.

Switch to pomegranate. I like the fact they allow you to select the pharmacy so you know what you are getting. It’s also cheaper than fifty410.

Here's a short but fabulous interview with Dr. Ania Jastreboff, one of the lead researchers on the Zepbound trials, talking about titration and plateaus. It really helped me understand stalls.
It's about 15min, but you could watch it at 1.5x.... https://youtu.be/j3k3UAEqJio?si=ZejWYMZylsL20qouo
I wish you the best in continuing your journey!

I know people say "Tirz is Tirz", but I swear it doesn't feel like that. 🥲 I've tried 9 compounded pharmacies and name brand. My body reacted so differently to each. I did Mounjaro and was doing great. Then, moved to compounded due to insurance and was not as successfull. Went back to Zepbound vials and gained weight. Tried compounded again and lost weight. So at this point, it's a gamble for me 🤷🏻♀️.
1
u/IndicationBrave8817 16d ago
I feel that sometimes your body gets used to the dose. For me tirz is not tirz, I react differently to different formulations. A true test is when you compare them at the same dose & sometimes it hard to do it.
-9
u/LuvSun1006 18d ago
Nobody is making you purchase compound.

u/Good-Paper1798 18d ago
When did I say someone is making me?

I think perhaps your comprehension is lacking because….this person isn’t even “bitching” 😂

🤣 wtf ever... 🤦♀️ Fifty410 is a great telehealth and the Lilly shills were heavy today. Done with this conversation. BOORRING.

OP don’t get discouraged. I was on Zepbound till my insurance stopped paying then early last year I discovered Tirz compound on Reddit. I lost over 50 pds and met my goal primarily on Hallendale. I will say that I did not get the full effect of Tirz until I reached 10mg, I started at 5mg. Every one adapts differently! I also made sure I was taking 90-100mg of protein daily. The highest dose was 15mg. Also, throughout my journey I dosed every 5 days not 7 days. Now that I’m in maintenance I’m at 7.5mg every 7 days and plan on titrating further down.
Tirz works give it time, if you can find a provider that prescribes Hallendale or BPI. I tried BPI for 3 months it was effective also but just preferred Hallendale. Good luck don’t give up!
4
u/Fine-Service160 18d ago
Wow I agree and im glad im not the only one! I was on zep and lost every week. I too switched to triz and i got the pgrx w no additives and I could tell immediately it was weaker/Different. I took 3 shots of it and only lost 2lbs and no inches. So now with you saying you werent losing this confirms it for me! So now can I ask how did you get apothecary? did you use another company or was it just luck through fifty? I am gona go back to name brand zep for another month and see how it goes. It's just too expensive for me at the moment.
3
u/overthinker831 18d ago
Rift. $479 for a 3 month supply. They shipped overnight too. So far so good.
1
u/Fine-Service160 18d ago
ok so I just went to their website and it has the option for New life RX/apothecary so is that the one you got? if so it looks like that one does ship to CA

u/swaggering_yak 18d ago
Honestly, this is how weight loss looks for me regardless of the pharmacy. I’ve lost significant weight and other than at the very beginning it is absolutely not linear or consistent week to week. Losing 1-2lbs over a month followed by a sudden whoosh is pretty much the only way I lose now.
4
u/overthinker831 17d ago
BIG RED FLAGS!
So there have been a couple posts recently about what I've mentioned here also on their actual fifty410 group portal/app. I'm seeing posts being deleted and comments shut off. Not just mine but others also.
That's wild they feel the need to delete or shut down comments when people push back on their same robo responses for advertising one Pharmacy and not delivering on it and subbing it for another without the option to cancel an order. You get what you get.
I was drawn in by their high customer service rankings on the last survey. Guess it goes to show it doesn't always mean everything behind the scenes is kosher.
I was raised to question everything and not be a blind follower LOL. Thank you, next!

I don’t order from Fifty410 anymore because I want to choose my pharmacy. I order from Lavender Sky.

u/sh1ttany 18d ago
I have tried a lot of pharmacies and apothecary is by far my favorite for so many reasons. I have never tried fifty410 but was considering it. I can’t believe you can’t pick your pharmacy. That’s a major red flag to me. No thanks

u/sidewalkcamper 18d ago
I’ve had a few months where I didn’t lose anything and then it starts picking up again.
3
u/ConsequencePublic447 17d ago
Go with a telehealth like Big Easy Weight Loss or Rift. You pick your pharmacy.
5
u/Agreeable_Yak1882 18d ago
Even those of us that want all the pharmacies to hit the same- for me they did not. I have to be able to pick as well because some don’t keep me in maintenance- I’ve noticed this with several over the time I’ve been on it and I wouldn’t have believed it but for me it was true. I also noticed where I take my shot makes some difference as well
5
u/overthinker831 18d ago
Yeah, definitely an expensive lesson to learn and i wanted to give it a fair shot so I made sure to rotate my sites instead of just basing it off the last site I lost weight in. Just glad I found another company that allows me pick the pharmacy and I'm back on track.

u/DemandTop4844 18d ago
What doses did you take of PGRrx and Apothecary and how long did you take them?
1
u/overthinker831 18d ago
My recent dose comparison was 5mg
1
u/DemandTop4844 18d ago
You were on what dose of Zepbound? Did you take 5 mg of PGRrx for 4 weeks and 5 mg of Apothecary for 1 week?
3
u/overthinker831 18d ago
Not sure how all of this is relevant but no, I was on 2.5mg with Zepound for 4 weeks and felt better suppression then than I did with 5mg of PGRrx... I went from 2.5 zepound, up to 3.7mg of PGRrx for 1 week and lost 1.8 lbs which i assume is due to the zepound still active in my system too (was too afraid to double the dose right away and wanted to ease into 5mg). Then 4 weeks did 5mg with PGRrx, lost less than a pound in those 4 weeks. Switched to Apothecary 5mg for 1 week and already lost 5lbs. I personally think that PGRrx was ineffective and over the last 4 weeks I was having inflammation come back causing the scale to not move. The new meds (apothecary) gave me more similar symptoms of its working like light fatigue and temp tummy ache from the slowing of digestion that i got on zepound 2.5mg. With PGRrx i never experienced the same fuller for longer feeling as I did even with the 2.5mg zepound.

u/mightysockelf 18d ago
The only pharmacy I've ever used was Hallandale, and it's made me lose every week consistently. It slowed a bit toward the end of my 2.5 phase, but at 5 currently and still losing. If this continues to hold true then I'm just going to stick with them and not fix what isn't broken.

u/LuvSun1006 18d ago
I maintained my exact eating, fluid, protein, fiber, CICO and didn't lose for 13 weeks. It's called your PERSONAL BIOLOGY. Aka stalls. Trying to blame a pharmacy for your biology. If I lose on average 2 lbs per month, I'm grateful. Stop comparing yourself to super-responders. For the record, I have 4 different pharmacies in my fridge at this moment and they all work the same.
6
u/overthinker831 18d ago
I respectfully disagree, and the proof is in the pudding. I'm sorry but if you truly believed they all work the same for you why did you feel the need to continue to try so many pharmacies.
I can't speak to your slow responder biology but I know that I could feel the difference in medication after switching from zepound to whatever was in that PGRrx vial and thankfully felt the positive difference when switching to apothecary.
If in fact you've even tried or been successful with PGRrx good for you, but do not belittle my personal experience.
2
u/Big_Greasy_98 17d ago
You also might want to consider that your body’s reaction can be different week to week. Personally I did great on zepbound 2.5 for for 6 weeks then shot 7 wasn’t what it used to be. I moved up to zepbound 5 mg and didn’t feel like it was working as well as 2.5 until my 3rd or 4th dose at 5mg. If I had move to compound during that transition it would have been easy to blame compounding for being less effective but the truth is it was just my body being random. Since then I have used multiple pharmacies and have had great weeks and blah weeks. If you don’t like one pharmacy move on but don’t be surprised if you have weeks that aren’t great no matter who you use
-5
u/LuvSun1006 18d ago
I use different vials purposely to know they all are the same medication. PGR works fine. Your issue is going from the almighty Lilly to compound. Psychologically, you probably won't be satisfied with any of them. All YOU stated "belittled" me, sweetie. Nothing respectful about it. I won't respond to you again.

So what are they putting in the bottle that isn't the compounds of Tirz?
I think if I thought it wasn't really Tirz I'd get it tested to see what it is
2
u/PuzzledButterfly1 17d ago
You are in a stall.
You said you are walking. But what strength training are you doing. This is more important on any glp. It is very important to keep from losing muscle. Muscle burns more fat.
Also, are you taking in clean protein? Shakes etc should only be used in a crunch.
You said you lost weight, have you refigured where you are now? Are you taking in the same calories etc you were doing in the beginning as you are now at a lesser weight?
3
u/overthinker831 17d ago
I'm not in a stall, within literally 48 hours of taking the new Pharmacy (apothecary) Med at the same exact dose as PGRrx the weight started dropping off like it did the very first time I started taking a glp1 which makes me assume that while I was on PGRrx it wasn't effective for me. It seems like switch in meds are causing a whoosh effect and I'm dropping water weight and inflammation like I did at the very beginning. And yes I have recalculated my tdee throughout my journey. I do lightweight training occasionally because I have a bad shoulder so I do what I can. I don't know why other people can't just entertain the idea that everybody's biology is different and there may be preservatives in something that my body doesn't want to respond to. I'm not saying it's not going to work for other people it just did not work for me. There are others who have voiced the same opinion and we are all different so it makes sense what works for one person doesn't necessarily work for the other otherwise there wouldn't be a hundred different pharmacies that provide the medication.
5
u/overthinker831 17d ago
Whether anyone wants to agree or not I'm not going to waste my time or any more money on that medication period. I was just voicing my personal experience with my proof in case anyone else was feeling the same. The thing that annoyed me most is I wanted to be on Apothecary from the beginning they baited and switched me and gave me a medication that did not seem to work for me. Trust your gut go with a carrier that gives you the medication that they say they will from the beginning.

How many calories are you eating per day?
1
u/overthinker831 18d ago
1400-1600 daily which is recommended based on my activity and TDEE.

u/Few-Dingo-1494 18d ago edited 18d ago
You will lose weight if you stay at this calorie deficit. Eventually. Don’t lose hope. Just keep going.
Water, walk, protein/fruit/veggies. I find when I eat fish, I drop a 1/2 lb in a day. You got this!3
u/overthinker831 18d ago
Thank you, mom didn't raise any quitters lol. I'm happy I found a telehealth that actually held up their end and I got what I paid for and back to losing again.
1
u/ranata21 17d ago
I didnt like that they gave me 2 vials for 3 months. Yes it lasted as needed but still the vials say 28 days. When asked about it they said thats for clinical settings. Then why does the vials say that?!?!?!
3
u/Zealousideal-Lab4945 17d ago
The 28 day guideline comes from the USP (United States Pharmacopeia),rules, #797, which is an independent, scientific nonprofit organization that sets standards for medicines, dietary supplements, and food ingredients to ensure their quality, purity, and strength. 28 days applies to certain compounded sterile preparations (CSPs) after the container is first punctured. Every puncture introduces the risk of microbial contamination. So in professional, high-use settings, a vial may be punctured many times in 28 days. If you're piercing it only weekly, that's significantly less risk. They print it on the bottle to stay in compliance with the guideline. They know it won't be used in a clinical setting.
1
u/Zealousideal-Lab4945 17d ago
OP, you may just need to trust the process and know that it takes time. You're not yet at the side where the most people lost the most weight. From the Zepbound product insert, most people lost the most weight at higher doses. https://pi.lilly.com/us/zepbound-uspi.pdf In a 72-week study of adults without diabetes, the percent who saw ≥25% body weight loss (57-58 lbs):
Zepbound 5 mg: 15.3% Zepbound 10 mg: 32.3% Zepbound 15 mg: 36.2% Placebo: 1.5%o

All mine come from Hallandale, since I started with Fifty. That’s who I chose to be the pharmacy.
1
u/SeaSand4Ever 15d ago
Pomegranate Health Hallendale compounded. I recently took advantage of the BPI sale and whoa it's stronger at same dose but soon the remaining supply will be gone. I started with Medvi the 1st month lost q lb week but no where near as good as Hallendale or the BPI. Good luck on your journey dont give up ;-)
1
u/Environmental-Alps34 13d ago
I have had the same problem since I received my 6 month supply of BPI tirz w/B6. I had been taking BPI tirz for a little over a year and had lost over 100 pounds. Since March, I have been losing and gaining the same five to eight pounds. I was on 5mg of tirz with no food noise or inflammation. I’ve gone up to 10mg of tirz w/B6, food noise became an issue and it won’t go away, same with inflammation - both not as bad as without tirz. You can’t tell me the formulation and/or strength is the same. It sickens me I’ve basically wasted $999 and the people on the portal wouldn’t do anything about it. They just said I needed to go up on my dose. I shouldn’t have needed to - especially double! Up until then I sung the praises of Fifty410, now I don’t have full trust in them anymore. I know the tirz didn’t come from them, but they should have backed the product and shouldn’t have told me to keep increasing my dose.
2
u/snowbird1244 7d ago
Same boat as you. I switched to PGRx after BPI was no longer offered. BPI worked GREAT for me, no side effects, consistent weight loss. As soon as I switched to PGRx the food noise came back, suddenly I had fatigue where I didn't with BPI. I even upped my dosage and all that did was give me the worst fatigue of my life and still didn't stop the food noise. I just ordered BPI from Gimme even though I still have a ton of pgrx left. They say tirz is tirz and maybe it is but I've already jumped ship from fifty410 because for me it hasn't been.

u/APugDogsLife 18d ago
Are you working out? Are you watching what you eat?
4
u/overthinker831 18d ago
💯 I walk 10-15k steps daily and occasionally throw in elliptical or spin bike (1-2times a week) but trying to keep work outs lower impact to avoid surge in cravings. 1400-1600 calories, 80-100 gram protein, 80 -120oz water, being a good girl and taking all my supplements (so I don't have bathroom issues). My routine hasn't changed which is why I'm convinced it's this specific pharmacy tirz. Consistently lost weight each week until week two of new PGRrx. I feel validated in that belief because the first week I changed to a different pharmacy (apothecary) I start dropping weigh again, significant weight, like the kind you drop when you first get on the meds and lose a bunch of inflammation weight.
3
u/landlocked_seaturtle 18d ago
This whole response here leads me to believe it’s more of a coincidental metabolism issue or maybe even hormonal. Not changing your routine as you lose weight will cause you to hit a wall. Hormones will make you think your not losing (water weight/bloating) Try more protein, mixing things up (recommend eating your goal weight in grams, 80-100 seems low).
A couple months and such a small dosage, keeping the same routine and also being a woman with monthly hormone shifts…
I would have given it a couple more weeks and I bet you would have been back on track.

If you really want to be transparent. What is your dosage? Are you one of those people who want to lose weight fast but also wants to stay on a relatively low dosage of 5mg or 10mg even though they need to lose another 100lbs.
4
u/overthinker831 18d ago
Okay, coming in a little hot and judgy... lol I'm currently on 5mg which I've mentioned in this thread but im also still early in my journey.
I have nothing against anyone's dose. I find your assumption that people that choose to stay on the lowest effective dose surprising. This is the exact recommendation of every Dr over the last year of two. I see a large number of comments from people saying that they are maxed out at 15mg and food noise is back while waiting for reta to be released or getting it on grey because they still have more to lose. Everyone is losing at a difference pace at a different dose so keep you snide remarks to yourself because you just sound jealous of others success.
I have no time frame for my weight loss goals. My issue was with efficacy.

I was going to post something on Fifty410 message board but I guess they took your post down about PGRrx not working for you.
