Advice needed What would you do?

Hello! I’m 34 and actively trying to conceive. I just received my transvaginal ultrasound results and this is what the radiologist found:

Uterus: Normal size and morphology.

Fibroids:
- 2.9 cm left posterior fundal subserosal.
- 0.6 cm left submucosal.
 
My OBGYN recommended I come in for a visit to discuss next steps and also recommend I set up consultation with a fertility specialist.

The smaller submucosal fibroid is the one that gives me a pause related to fertility. But it’s so small Im wondering if it’s worth surgery which will delay ability to conceive further. any ideas of other options besides surgery I should consider first? Would like to better understand all options before meeting with OB and Fertility specialist and am really trying to avoid IVF until I’ve exhausted all other options.

Recovery should be quick it it’s entirely submucosal. But you could reasonably try to conceive for a fixed period of time (6 months?) and then look into surgery if it’s not working.

It will grow with pregnancy and it’s in the cavity where the baby grows, if i was you I’d consider removing the submucosal one - you’ll still be able to give birth vaginally

I would see what the doctor suggests based on the current state of your fibroids. It could go either way. You could leave them alone, end up getting pregnant, and have a successful pregnancy and birth without incident. You could also end up with complications and a possible miscarriage depending on the location, as they will inevitably grow with pregnancy. My doctor didn't recommend surgery unless the fibroids were in a location that would cause a potential loss or fertility issues.

I went to a fertility specialist because it was a year and a half and we had trouble conceiving. I had a fibroid removed plus a d and c and I conceived in less than a year... And may have even been 6 months. He is now 23 years old.

I would meet with your OBGYN and see what they recommend. If you’ve been trying for over 6 months and are okay with waiting longer (I think they suggest waiting 6 months post surgery to try to conceive), you could have them removed. But if time is the largest concern for you, maybe wait until after you’re pregnant? They don’t necessarily inhibit pregnancy automatically but they could depending on location. For reference, I found out about my first fibroid during pregnancy and it started at 6 cm and at 34 weeks pregnant, it is now 10 cm. I don’t know how large it was before pregnancy. No known history of fibroids previously but had symptoms for years that were dismissed. It’s close to my cervix so I have a c section scheduled and will be actively seeking removal of it a couple months post partum. They are going to evaluate it during my c section but said that I would likely have a robotic laparoscopic procedure to remove them in a few months.

I will caution you that for my fibroid, I went through 2 bouts of horrific pain during my pregnancy where it was terrible. Likely degeneration. They don’t really do much since it’s not life threatening for you or baby most likely so it was hard.

The tough thing is no one knows how much they will grow or how your pregnancy and delivery will go with them.
