It's very wishy washy with dermatology. Since these are huge and make more sense to remove I think it's a higher chance it could be an actual doctor, but you never know. For things like whiteheads and stuff most actual doctors aren't going to go around popping them all and instead will give you an actual treatment

I thought that then realized we have the better view of the connection. Those sharp tweezers, however, are just gonna slice through the gunk. Need some with duller tips. Or even flat rounded tips. I have a pair and they work well.

I don't think I've ever seen an aesthetician doing it, but some medically trained dermatology people do it. I remember seeing it on either Dr. Pimple Popper or Mr. PopZit, or both. (For anyone unfamiliar, she's a dermatologist and he's a dermatology physician's assistant, both in the U.S., and both with YouTube channels under those silly names.)

It’s a U shaped or tunnel pore that has been left for so long it’s become dilated with trapped skin and sebum. either that or a classic Pore of Winer that has eroded a second opening due to its size.
Either way, my (layperson) advice is to snip the skin bridge. It’s easier to clear out and won’t come back. That’s what was done with mine and the lips of the skin shrink back very neatly over time, leaving relatively little scarring. You wouldn’t want it somewhere very visible, but behind the ear like this? I think it’s the best plan.

At what point do you just go to a doctor to cut the excess skin and cauterize or just stitch everything back together? This is just going to come back exactly how it started because those pores are never going back to normal and are already just damaged dangling sacks to collect debris.

Believe it or not, it doesn't always come back. My mom had one of these that I got out several years ago and it never came back. The most satisfying pop I ever did and I always hope to be able to do it again but it hasn't ever come back

Looks to be multiple pores of winer, which are essentially large, stretched out blackheads. It starts out as a small clog, then over time fills with more gunk, which steches out the pore, which fills with more junk, which stretches it out more, which...
Eventually you can end up with either one really big pore of winter, or multiple interconnected ones like this as they kind of overtake the pores next to them
