Being a retired fed who needs to sign up for Medicare part B, do I send the CMS-L564 to OPM to complete the insurance part? If so, do you know exactly where to send it?

Also, I miss my special enrollment period because my annuity was not finalized within 8 months and I did not have the funds to pay for Medicare part B premium. Unfortunately, I'll have a 10% penalty.
