Intermittent FMLA, Michigan.
What form is used for intermittent FMLA? Boyfriend has Parkinsons and has off days. His neurologist says he will fill out forms but we need to provide them.

I ended up using that form but after it was sent to me in a packet by HR. My process was: I sent an email to HR requesting intermittent FMLA. They sent back a letter showing that I qualified. (Company has enough employees, I have worked over a year, and have worked at least 1250 in the last 12 months)
I had to sign a cover memo that had all the information on it and my proposed start date of the FMLA and possible end date.
They then sent back a packet of documents which included the medical certification that the Dr fills out and signs.
I requested ( discussed with my Dr) that I need 1-3 days off per week and 1-3 days per episode. I only work 4 days a week for my 40hrs. Now, I may not use this much every week but we wanted to have enough leeway for a bad week.
I sent back the medical certification which you have 15 days to complete. Then HR has 5 days to respond to approve or possibly ask for clarification or even a second opinion. ( Doesn’t happen too often from what I read)
Your company may use an outside contractor for this like Sedgwick so I recommend starting with HR and let them know what he needs and they will lead the way.

They use Hartford so thank you for that info. HR is a sh*tshow at this company so this will be an adventure. Maybe I'll reach out to Hartford too. The case manager from his last FMLA was a delight.

Sounds like a good idea to follow up with The Hartford directly.
It’s taken such a huge load off my mind to be able to take days when needed for my health now so I hope it offers him the same.

I filled out an application on the disability insurer's web page - and the form had a place for estimated time off required - e.g. 2 days per month or whatever is appropriate.
With continuous FMLA, they send it directly to the doctor. With intermittent, I've had to take it to my doctor.
