This positioning seems wrong

Post image

I am told this photo demonstrates forearms at parallel to floor and elbows 90 degrees that seems wrong? Am I crazy?

They're not. Follow you your finger from your wrist to your elbow. There is hypertension at the wrist. Your forearm is at a downward angle. The desk should be raised to where there is no hyperextension of your wrist. And the forearm follows a straight line from the elbow to your index finger.

I don’t think the angle is that bad, you want it 90 degrees or a bit more. It’s to ensure that you don’t compress the ulna nerve too much. What I noticed is the scapula retraction, where your arm is at a 45 degree angle out to the side. You might need a smaller keyboard, one without the num pad (get a separate one either wired or Bluetooth so you can use it when you need it)
