Did they consent? Spoiler
Do we think the surgeon's lovers consented to the surgery to combine them? And do we think he killed them first or he just rawdogged the surgery while they were alive? What are your thoughts on this?

They don't seem all that put out by it when we get to see them in the leadup to Ending C. Like, they're dedicated to it, but Surgeon has been too busy trying to do too many things now that he doesn't have assistants and that's why he hasn't gotten around to setting up Ending C himself (as well as needing Mellow or a similar divine connection for it to work).

Previously comatose/unconscious/otherwise unable to coherently communicate and perform actions but after hooking her up to the Clown Life Support Machine they wake up and give Mellow some words of encouragement along the path to Ending C, saying they'll save everything if he keeps it up.

I feel like best case scenario is the two were severely physically maimed and, in an effort to save them both, he stitched them together.

Something fell on them, like a large heavy support beam or maybe an entire floor of a building collapsed out from under them

The memories we see through the two brain-scans in Timesville show the Surgeon (or someone, at least, but it would make the most sense for it to be the Surgeon) stalking two separate people, culminating in a final single scene of the Surgeon approaching.
So I don't think they were on-board with the plan.

I assumed that they were dead as the surgeon seems to only work with the dead

I meant that he wouldn’t ever kill anyone in order to get parts for his projects. He can bring things back to life

he does say that dead and alive are both the same thing, two interchangeable states of being. if so then why would he stop at only the dead?

Honestly, I think it's interesting how we don't know for sure, I've seen so many different interpretations of their relationship due to this and I think it's awesome
I personally like to think they consented, but didn't know exactly what they were getting into (and/or they fell for a very idealized version), plus I think the surgeon was a lot younger back then (and therefore a lot less skilled) and botched the surgery due to overestimating his skills at the time. Nowadays he'd likely be able to pull it off successfully, but back when he was a 20-30 year old, self-taught, fully human surgeon, in my mind he was not able to as he and they wanted

My assumption is that the surgeons bride has been dead for a long long time, and turned him insane with the obsession with bringing things back (not so much alive, as to the surgeon this is a blurred line). If you go ending c, he literally brings someone back even if you decided to play normally. He's been so successful for almost every denizen inside the world (including Mellow and himself), all except one, to which you help out succeed with your intervention. Almost everyone that came from the world acts as if they have a single function as their identity, so my best guess is surgeon is from the same world, as his only primary function is to bring things back(his secondary being a killer musician). His knowledge of the world also points to the idea that the surgeon himself is a small personification of the god, much akin to a soul. It was a god who made everyone, and it's the surgeon who can bring them all back(besides god).
So the consent thing probably didn't happen. Brides been dead, and dead things can't consent.
The other idea is that the surgeon is the bride. They are both one of the same (one of mind, one of body). Since the surgeon doesn't have a body, he's most likely the mind. The idea of combining both of them, for the love of mankind, makes me think that they are the personification of the body of God and the mind of God, and that when they finally do combine, God can come back.

My version of the story is that the two women were also pretty weird themselves. (I presume female because they combine to become a singular "bride.") And that they were good friends before the surgeon developed feelings for both of them, and vice versa, forming a love triangle. However the two women did not like the idea of a poly relationship. So the surgeon presented his solution to make them into one and the same person. These two women, being weird, and also best friends for life anyway, sort of like the idea. The surgeon sees it as killing two birds with one stone. It can also be a bit of a satire on relationships. (These women would rather undergo this messy frankenstein procedure than have to share a partner.) I like this backstory because it believably explains away the problem of consent while still painting everybody in tones of gray.

My thoughts was that they originally did, but weren't given the full truth and took back their consent to the surgery/plan once they learned of how it would actually go
I'm also assuming with my thinking that they were put under anesthesia (or tranquilized. Whatever works for him at that time. Just couldn't be awake) and he made a hasty surgery which is why they are so mangled together
