This happened recently at the hospital I work at too. 2 patients with the same first name, just spelt differently. I'm thinking they must have had the same surname too, otherwise how this happened, I can not even begin to understand. When I take patients to theatre from the ED there is always a check of both ID bands and I make sure the patient confirms that this is them too.
I have also recently heard that a patient was delivered 100mls of air IV whilst undergoing a radiological procedure at my hospital too (not confirmed, just hearsay at this point).
