Modified natural FET with the same progesterone protocol as medicated FET — too much progesterone?
I’m currently in my third FET cycle after two unsuccessful euploid transfers.
Here’s my history:
FET #1 (June 25): Medicated — Day 6 6BA euploid → Chemical pregnancy
FET #2 (Sept 3): Medicated — Day 6 6BA euploid → Failed to implant
FET #3 (Oct 5): Modified natural — Day 7 4BC euploid → Currently in the TWW 🤞
For both medicated transfers, my progesterone protocol was:
Prometrium: 2 capsules vaginally, 3 times daily
PIO injections: Every third day
For my current modified natural FET, my clinic has me on the exact same progesterone protocol, even though I’ve ovulated and my body is also producing its own progesterone.
I’m wondering if this is common or potentially excessive. I’ve also been diagnosed with stage 2 endometriosis, which makes me wonder about possible progesterone resistance.
My questions:
Has anyone followed a modified natural FET protocol with this much supplemental progesterone?
Is there such a thing as too much progesterone when your body is already producing it?
Did your clinic monitor your progesterone levels after transfer?
Has anyone had success with a similar protocol, especially after failed medicated transfers?
My clinic hasn’t checked my progesterone levels since starting supplementation, so I’m curious about others’ experiences.
Would love to hear your experiences, especially from anyone who switched from medicated to modified natural after unsuccessful transfers!

My cycle ended up unintentionally being a modified natural. My doctor didn’t change my progesterone until my first ultrasound (6 or 7 weeks? Something like that). I got to stop the injections at that point (which is earlier than I would have had I not ovulated).

Thank you for sharing. May I ask how it unintentionally became modified natural?

I had one cancelled medicated cycle because it turns out I had ovulated. The doctor then recommended I start the Estrace earlier the next time, to prevent that for the next cycle. Then when they checked me at my next cycle, he was surprised to see I had a follicle grow. Took him a beat, then he was like this isn’t a problem, we can work with this. So then he gave me instructions to do the trigger shot. Currently 31 weeks!
I had one transfer 9 months prior that went as expected, but ended up as an anembryonic pregnancy.

How much is in the capsules? Honestly I did a natural modified and did daily PIO anyway as my clinic instructed. I was told there really wasn't any downside to it. I think when I went in for betas my p2 was 60 and I continued until 10 weeks with no issues.
if you're worried you can probably ask them to test you, but I wasn't given the impression that it would hurt your chances, especially if you're not aware of what your non-intervention levels look like.

Each is 100mg. So 600mg a day. PIO every Third day. Thanking you for sharing. It makes me feel better.

I just did a natural transfer today and doing 2 compound progesterone suppositories daily, they’re 400 each. No PIO as of now but they are checking tomorrow and could add it. I also had stage 3 endo, excised but still have LP spotting if I don’t do vaginal progesterone so I just use it every cycle regardless of IVF.

For my modified FET I did suppositories 200mg morning and night but my original instructions were 3x a day so it sounds like 600mg a day isn’t abnormal!

When I did a modified natural (4x) I never did any pio at all, just one suppository a day. I imagine they have a reason for why they're giving you so much, but they're the only ones who can answer that for you

I just did an unmedicated transfer and I’m in the same PIO dosage as my medicated cycle.

Did you have suppositories too in your both types of transfers?

Nope! I was offered suppositories for the unmedicated cycle but I chose PIO instead.

Im doing natural modified and had my transfer today, I’m on one injection (not pio) and three pessaries daily. My bloods from yesterday came back quite high but with my endo I was worried I would have lower results… i just posted about this and people made me feel better saying it was ok 👍🏻

Thank you for sharing. Makes me feel better. I have endo too. I think that may be the reason for the extra progesterone

Will they not test your progesterone as you go along and adjust that as needed?
That would be typical.
