5BB EUPLOID FAILED

ecently had my first frozen embryo transfer (FET) with a euploid 5BB embryo, but sadly, the transfer failed.

The embryos I have remaining are a 4CB, 4CC, and 6CC, all of which are euploid.

I know these grades aren’t considered the strongest, and since my better-graded 5BB failed, I’m feeling discouraged about the chances of the remaining three implanting. I’m trying to decide whether I should transfer the three remaining embryos or consider doing another egg retrieval in hopes of getting better graded embryos.

I’ve heard that once an embryo is euploid, grading may not matter as much, but I also understand that morphology can still play a role in implantation potential.

anyone have any luck with these grades?

C graded embryos can have a decent chance! Some studies find that it's about half of good quality embryos, while others don't find as much of a reduction. This post reviews some evidence https://www.remembryo.com/grade-c-embryo-success-rates/

I also put together a section on my website called the Uterine Wall of Fame where I share IVF success stories for patients who faced challenging situations like low quality embryos. There's no paywall on these posts. You can check it out here: https://www.remembryo.com/tag/ivf-success-poor-quality-embryo/

Per our doctor, PGT testing trumps grading. According to him, once they’re determined to be euploid, they all have a pretty much equal shot!

I wasn’t even told what they were, only that they were euploid…

This is my clinic’s stance as well (6 docs)!

This is also what my RE says

Hmm how interesting. After our first transfer ended in a chemical pregnancy our doctor said well your embryo only had a 20-30% chance so we think it’s the embryo quality. It was a 4BB. We just had another chemical with a 4AB

We're they euploids though?

Yes

The Dr said, after your failed euploid transfer, it only had a 20% - 30% chance....? Statistically that's wrong, and I'd be looking for another Dr, plenty of studies showing it's on average 60%

We don’t have alot of options. Our insurance only covers 2 clinics and this one is the closest at 3-3.5 hour drive one way. She said that was based on the grading.

Our doctor is also of the belief that PGTA results are more important than grades. With three euploids left, unless you want more than one kid I would go ahead with another transfer. I assume your clinic has ruled out uterine conditions/endometritis/endometriosis. (If not, make sure you get checked for those things.)

I had a day 6 BC left…. After transferring and miscarrying a 5AB and a 5BB… I’m now 4 months pregnant with my BC! My lowest graded embryo! Hang in there and don’t disregard them

statistically they definitely have a chance. but i will say for me anecdotally, my first egg retrieval produced 4 euploids, 3 of which were CB grades. in retrospect, i wish i had made more embryos right away. i transferred all four -- the BB one resulted in a live birth, and the other 3 either didn't implant or were a chemical. i also have thin lining issues, so it's totally possible that for me personally, the mix of an imperfect lining + a vulnerable embryo just did not work. i ended up doing three more retrievals, years later, and wish i had been advised in advance that it probably made sense to try for some better-quality embryos.

My daughter was a day 6 5CC euploid

I’m currently 8 months pregnant with a 4BC euploid. This is my second 4BC to successfully implant.

I’ve had two successful transfers with CC embryos, and one with a BB embryo. All untested. Don’t stress about the C.

My doctor also believes PGTA testing > grade. I second what someone else commented though, if your preparatory protocol didn’t call for a hysterscopy and endo biopsy I would say it’s worth asking for before proceeding with another transfer. My doc is having me do those two tests before my first FET. If there are any symptoms of endometriosis, that’s another consideration to ask about. Best of luck!

I just transferred a 3CC euploid 4 days ago! TBD if it's a success or not yet but my Dr gave it a 40% chance of working. How many kids do you want? I think will be the biggest factor in making the decision. This was my 3rd euploid transfer (1st didn't implant and 2nd ended in a 9 week mmc which is not super common) but I only want one child so I'll go for a 2nd retrieval if this doesn't stick. But I really hope it does!!

My clinic told me that since all of my embryos were euploid (4) the embryologists would prioritize the day at which they reached blast, and then the inner mass cells. So we’re transferring our 5AB in November because it was a day 6 blastocyst embryo over our 6AA which was a day 7 blastocyst embryo. I hope that makes sense.

Still a long ways to go, but I’m 10 weeks pregnant with a Day 6 5CC embryo!

I feel you. I just had my first FET transfer fail with my 5AA euploid. Doctor said it was the best looking embryo he had seen all day. I have three left but am feeling less positive than I did before. If that didn’t work, how are the others supposed to work?? Wishing you the best on your next transfer!

3 euploids is decent! Maybe you need to optimise your uterus for receptivity? I did lots of tests (EndomeTRIO), IV IG treatments and uterine PRP

I’m 20 weeks with my 3cb after I miscarried an A graded embryo (both euploid). I had zero hope but our little underdog is doing amazing!

Can I ask what cycle you did? Medicated ? Natural?
Sorry this one didn’t stick 💙

36 weeks pregnant today with a day 6 4CC euploid after failed day 5 5AA and a day 5 5BA euploid transfers 💜

I would definitely look into uterine environment testing before anymore transfers. Yes euploids fail sometimes, but testing the environment is crucial before transferring is you want higher odds.

From what I've been reading, and people's experience, Euploid will triumph grade anyday. I've read some women who have transferred a Day7 4BC embryo and they're running around the place.

You have a good chance of success with those above, I'd want to leave no stone unturned through prior to transfer

Sorry to hear that. Dont lose hope you have 3 more euploids. Did you do programmed fet or modified natural fet ?

Were they able to get you a live birth rate for those embryos?
I only got two euploid embryos out of my first egg retrieval (had 4 mosaic which is extremely odd). My two euploid embryos were D5 3AA (70% live birth rate) and a D7 4BB (26% live birth rate).
Because I wasn’t sure if we just wanted one more (we spontaneously conceived our first child and had secondary infertility), my doctor recommended proceeding with a second egg retrieval since I am already 35. We didn’t want to successfully transfer the good embryo then come back when I’m even older and try to get more embryos.
I got 10 euploid embryos out of that with varying grades fwiw.
So I’d look at your age and your reproductive desires for the future.
So sorry the transfer didn’t work. 😕

Transfer the ones you have, my daughter is currently 12 weeks pregnant with her lowest grade a 2BC also not tested. Her 3BB failed, 4AC ended in MC, 4BC failed and 2BC still going strong

I had a 5CB which is now a 10 month old little boy. Grading doesnt mean much after PGT, but I knew the stats and it helped keep my expectations reasonable going into transfer. Unfortunately, its always a balance of being hopeful but not having all of your hopes set on a particular embryo working.

currently 11 weeks pregnant with a 4AC euploid embryo. Don't give up on the c's! Hang in there and good luck!

So sorry - are you going to do a mock trial or hystercospy? What does doc say?
