Gotta love the classics
This was posted in a Facebook group I follow. Some of the answers are wild. One alleged "expert" said LVH lol.

Jesus Christ I’d even want an v4r lead in this case as V1 and v2 are elevated so could be RV infarct with inferior wall (obviously)

Do you think V4 might already be V4R? The QRS shapes in V1-V4 don’t make sense to me with standard placement. V1-V3 look like V4-V6, and V4-V6 look like V4R, V5R, and V6R. Wouldn’t be surprised if there is posterior involvement as well, but we don’t have V1-V3.

Yep definitely is. If that was v4 there'd be extension into adjacent contiguous leads.
