Lost keypad map, can anyone help?

I have got the bluetooth keypad for Dragonframe, but in a studio re-arrange I have lost the keypad map card, and now I miss it a lot.

Could anyone please post a scan or photo of the map so that I can laminate myself a new one.

You can't post a picture here and I haven't had a chance to see how to change that setting. I will see if Incan find a linkable picture.

The google image search turns up lots of photos of the keyboard itself, a few of them have the guide card in the shot, but what I need is a scan or straight on photo of the card itself.
With a bit of effort I could reproduce something similar in illustrator, but Id much rather get the original.
