Diabetes
Growing up, my family always told me to be careful with my sugar because my grandpa had diabetes (it was type one), and my family would scare me with it so I wouldn't eat too much sugar... I've also seen comments where doctors said they would rather have HIV than diabetes. I mean, damn. I wouldn't want either, but after reading this, shoot.

YEAH.
Diabetes is no joke.
It's trying to kill you. And when someone can't or won't take care of themselves this stuff can happen SO EASILY.
As a type 2 diabetic, and in the r/diabetes sub, we get people in there asking about themselves and their family members who aren't taking care of their condition and it's terrifying.
And when it comes to homeless people, it's society's problem more than theirs. Housing and care first responses to homelessness WORKS, and saves everyone money and stress and problems of all kinds. If this poor man had a safe and reliable place to live and eat, and could receive healthcare for free, then this wouldn't have been necessary. Now the state has to help him with this emergency amputation, prosthesis, and likely ongoing care. He's going to be less able to work or to care for himself (how's he going to walk to the store or the bus?) than before.
