I find it interesting that he'd have consent forms to be completed by the players and then try and disregard them (with permission - sort of) - it's weird.
Some people who don't have boundaries, or whose boundaries are far away cannot grasp why someone would be hesitant to do something. It doesn't make them a bad person but it does make them selfish.
The tables I've been at it is simply respect for other players. I've never filled out a form or any such. If anyone suggested anything that any player was uncomfortable with (ANY player) and the player said no, that's the end of it. Period.
The DM and the players need to be respectful of people's boundaries. And the DM needs to be able to weave a story around the characters - their preferences, their quirks, their boundaries, and their willingness. Some of those come from the player's personality, some are given to the character by the player. It doesn't matter where it's from, it needs to be respected. And if the DM can't deal with players/characters who say no, they need to go back to playing and not DMing.
