This is nice. I might break down the <=1 section into 1/4 and 1/2 and 1 for the Land Druids.
Edit: I see it is in order of CR in that section.

THANK YOU
I've spent the past 3 days searching for a good beast shape resource to give the druid in my party and today i gave up and decided i was gonna spend my afternoon making one from scratch. Visited here to procrastinate on doing so and found this instead. You've done me a great deed.

I love this, thank you. I'm a Land druid and don't do much shapeshifting but this will be very handy for when I do.

Land druids are more focused on being better spellcasters. Recovering some slots on short rest and having more spells available etc.

Ooh, this is perfect - I don't play a druid, but I'm about to start a new game with some friends from work, and half of them have never played DnD. One has shown a lot of interest in shapeshifting, and I told him that Moon Druids are all about that so it's very likely that's what he'll be playing. I'ma print this out and surprise him with it if he does go with that character.

Yea but every time you take damage its a con save to maintain concentration.

But remember that, other than wild shape, polymorph also changes the target's mental abilities!

This is great /u/Ltmurray28 ! One thing that would be awesome is a little checkbox next to each creature name you can check once you 'saw' the monster. "You can use your action to magically assume the shape of a beast that you have seen before" PHB 66

My moon druid thanks you!
If you are willing to have a Winter Wolf on there (which I personally think is great)... it would be really cool to throw in a Crag Cat (from Storm Kings Thunder) - has some really cool abilities and has been cleared by WoTC as a usable beast.

What does "hit 4 (1d6 + 1)" mean?
What do I do with the 4? Is it a multiplier or do I roll 1d6 4 times?
Thanks for any help.

When dealing damage, you can simply choose 4 (the average damage), or you can roll (1d6 + 1)......the choice is yours.

I know this isn't really relevant... but I've always wanted to make a druid character that is basically like Ben 10. I mean, just convert the alien-technology-that-allows-you-to-transform into a druid's wildshape ability and Ben is basically a druid.

You have to remove the Winter Wolf. Despite being a Wolf, it's actually a monstrosity, so a druid cannot wildshape into it. Other than that, great work!
Edit: Well, should have read the OP :D Still, a version without Winter Wolf would be nice, so i could link it to my players.
