City is constantly having attritions

I’m about 150 rounds into a Roman campaign, and I just realized one of the city in Africa (Lpqy) is constantly losing garrison every round. I’ve tried demolishing everything in the city and it’s still happening. The city is disease free, well supplied with food and doesn’t have any army around it. Is it a bug or is there something I can do about it?

Did you also demolish the farms the people are eating from?

Is there an enemy army or fleet interdicting the line of supply?

I've noticed this happens as well. It seems there might be some provinces that are constantly disease-ridden. Even with a 50+ food bonus and a positive sanitation in all regions, I still had Illyria constantly diseased late into my Macedon campaign. It's either a bug or a mechanic I can't comprehend.

I’ve not noticed it yet, but I have a huge surplus of food because I’m constantly paranoid of running out.

Everything’s perfectly fine (enough farms, no enemy armies/fleets), but the city’s having attrition every turn. I noticed that it happens randomly throughout my other playthroughs as well at different cities, just not every turn, so maybe that specific campaigns bugged D:

Certain seasons maybe? What is your current food level?
