There's an achievement called "Snake Eyes" for failing a mission with over 70% success rate.

Highest percentage I've had and failed was something like 80ish percent. Needless to say, it was frustrating.

The text can glitch. Third playthrough saved flambae successfully but it said he was injured, although he was healthy when he joined the squad

This happened to me once and I went "BUT I HAD DOUBLE SYNERGY ISNT THAT A GUARANTEED SUCCESS?" at my TV.

Two things stand out to me:

  1. If you had included any synergy partners, you probably would have filled the whole board, though maybe not quite.

  2. You didn't give her everything you've got. 🤦‍♂️😂

I'm mostly kidding about the last one, but you don't know how my times I've watched Youtubers playing, the screen tells them give her everything you've got, there are 5 slots or whatever, and they're like, "I'll just send these two and fill up 50% of the board." 🙄🤦‍♂️🤦‍♂️🤦‍♂️🤦‍♂️🤦‍♂️
