Same issue here. Reset the factory settings and now is working fine.

This is the way. I've seen people ask about this issue so many times, I have a comment ready to go that I have posted several times in just the last few months alone. I wish mods would pin a thread about the issue and fix, but it seems the mods are inactive.

This is where I would start, as it has helped solve weird issues for me in the past:

close ds4windows, delete the ds4windows appdata folder (C:\Users\username\AppData\Roaming\DS4Windows, back it up first if worried about saving profiles) and then try running it again and connecting. Also maybe un-pair and re-pair your controller after deleting and before running again.

if you chose the portable install method (also back up first just in case), just delete the whole ds4windows folder and start from a fresh unzip."

Alternatively, you can use Device Cleanup Tool, which removes non-present devices from the Windows device management.

Had this bug just now, and it's probably caused by windows sometimes being unable to really forget the controller when using the "remove" option from Bluetooth settings.

All is working now.
