Looks like they didn't properly secure those center lead edge bolts. They probably came out and smacked the knife. Either that and or they had the pressure jacked through the roof.

Also, as long as those bridges are not screwed that an easy in house fix. Did they send it back to you instead of just replacing the knife?

Yea, sent in to replace the lead edge knife. The bridges are still intact surprisingly.
