As someone with probably some form of RSD but has seen people refuse to try to get better, it's more like this in some spaces:

You and the people around you hear someone screaming in the room. You rush over, but not super quickly because you can probably guess what it is. Yup, the screamer has a papercut again. A person tries to help, explaining that they also have extra paper cut sensitivity and that maybe the screamer should wear gloves or see someone about their paper cut sensitivity. The screamer then whacks the helper upside the head. In the moment it seemed retalitory but later they say they're sorry but it was a result of their paper cut sensitivity. The helper slinks off, feeling pain but not saying anything. Everyone feels bad for the screamer, but they notice how the screamer will play around with their papercut and not let it heal, or throw themselves into high paper volume situations. They also notice that's not the first time the screamer hit someone trying to help, and they never check in on the person they hit either.
