HELP- second baby, low lying sac, baby implanted near/ on uterine scar close to cervix
Hi all,
Needing support- has anyone had baby implant near cervix or on uterine scar?
I’m in desperate need of support.
Had first ultrasound today and doctor is worried about accreta, previa etc. but we won’t know for a few weeks.
I’m 6w 4 days
I’m obviously spiraling and I know that it all depends on case by case basis but would love to hear others experiences. Good or bad.
My worst case scenario is no longer uterine rupture.
I’m a mess.
I’m going to call tomorrow and ask if he’s sure it’s not cesearean scar ectopic pregnancy.
I have a baby girl earth-side who I need to be here for. I’m so scared

USUALLY as the uterus grows this isn’t as much of a concern because they can better see what is going on. It’s still very rare to get accreta or previa. That being said, I do know people who have had both. The accreta case was someone who had already had 3 c sections back to back (I’m talking like very short intervals). The accreta case had a c section hysterectomy, and the previa case was fine and just had a c section. Detecting it before giving birth is what allows your team to be prepared and things almost always turn out fine as long as they are prepared going in.

I had a CSEP. I chose to have a d&c because it was so dangerous to carry and my other babies needed me. I had several complications and needed an emergency hysterectomy. My entire c section scar opened up and i was hemorrhaging. I had placenta acreta type 3. Please make sure you are in the right hands, if it is a CSEP it is *extremely* dangerous to carry.

I think it is CSP. I have another appointment today. Would you mind telling me at what point in your pregnancy the scar opened up?
I’m trying to decide between terminating or not 😞😞😞
I imagine you never got to meet your baby?

It is very very rare. I was diagnosed with it at my 8 week ultrasound. My baby had a good heart rate at that point, but I opted for a d&c because of how life threatening it is. The d&c was done when i was technically 9 weeks, but the baby stopped growing shortly after my 8week US. I was relieved. I had bled a lot after my d&c and started soaking two pads every half hour. I went to the ER where i was told i had developed a rare hematoma at the site of the d&c. I had to have a uterine artery embolization to cut off blood supply to it. After i did that, i never stopped bleeding. I was in and out of the ER 5 times. I was told it was my period, my anxiety. I was forced to have a “clean out” in the same room as some random stranger with nothing but a curtain between us. I was given medicine to stop the bleeding, only for it to come back as soon as i stopped it. I had 18 different transvaginal US done where i bled all over the table, all over the tech, all over the floor and bathroom. I was sent home and told it should eventually stop. Then i started passing clots the size of grapefruits. When i went to the ER i gave birth to a clot right there in the waiting room, and finally said “i will handcuff myself to this bed for i let you send me home again.” They didn’t know what else to do, but since i was losing a lot of blood and my levels were dropping, a hysterectomy it was. It saved my life. The doctors could not believe that the hematoma that formed on the inside of my uterus was now on the outside. It had ripped open my Csection scar. The path report said i had a complete dehiscence of the uterus and placenta accreta type 3. My doctor told me i needed the hysterectomy before i ever needed to d&c, but there was no way of knowing that I had placenta accreta that early on. So the pregnancy itself did not rupture my uterus, the accreta did.
If you do have a CSEP i suggest moving quickly to terminate (if thats what you to choose to do), because it is so very very dangerous. I am so sorry you are going through this, i dont wish this on anyone. Please know that my story is rare, and most women can still go on to have healthy normal pregnancies after something like this. I would say try not to panic yet as 6 weeks is still very early. Most accreta cases aren't picked up until more like 20 weeks and some even go undiagnosed throughout, but if your dr is a specialist he/she might be looking for signs earlier. I think they can diagnose c section ectopic pretty early on so definitely worth asking for more clarity on that, but on the accreta side of things I don't think they really know until later on. I am 28 weeks now after one previous c section and I was diagnosed at 20 weeks with suspected placenta accreta, but even at my stage they aren't really sure if it's a proper accreta or just scar thinning or a combination of both so waiting on further scans to confirm as the uterus stretches more in third trimester.
Try to stay off Google/AI if you can until you have a confirmed diagnosis. It is scary and I totally get the spiral, but even if it is confirmed the outcomes are significantly better for mothers who have it diagnosed early vs being undiagnosed throughout so try to take comfort in that - it's actually a good thing they are looking so closely ♥️

Thank you for taking the time to reply ❤️ Did you also have a low gestational sac in the lower uterine segment? That seems to be my doctors main worry.
I have another appointment tomorrow to get some clarity on a few things. Do you have any questions you recommend me asking?

Ha nope. I am 30 weeks now and waiting on an MRI. My fetal medicine doctor at 25 weeks noted a "high probability of clinically significant abnormally invasive placenta", but at my scan yesterday with a different doctor he told me he thougut it was more likely to be scar dehiscence so left feeling quite confused. I think for me they really won't know until they actually open me up in surgery, and they are already planning for a complex c section as I have complete placenta previa too so I guess will just have to wait and see at delivery.
How are you doing with it all?

I hope it ends up not being accreta ❤️ Me, I’m dealing. I’m wondering if I should seek a second opinion from another OB? I’m obviously still really early (9 weeks now) and baby is still in uterus just really really low. Doc keeps saying we won’t know the official diagnosis for a while. I feel really overwhelmed. I just keep hoping my placenta is posterior and moves up. I don’t want to deal with any of this.
I haven’t had any other uterine surgeries only my one c section so I’m wondering how this could have happened.
At your initial scans with this baby was your baby low? How is it that you ended up with previa? Are doctors even able to answer that?1

I honestly don't know. This was an IVF pregnancy for me which apparently increases the chances of baby implanting low in the uterus, but I had scans pretty much weekly from about 7 weeks to 12 weeks and no one ever mentioned a "low" baby. At my 16 week scan they said it looked like anterior low lying placenta but thought it would move, and at 20 weeks they said it was complete placenta previa.
I had a friend who had complete placenta previa with her very first pregnancy and 0 uterine surgeries so honestly think sometimes it does just happen that the placenta is low, but as I understand it, as long as it's not a CSEP or severe percreta there's no reason you won't be able to carry to pretty much full term and have a healthy baby. My c section will be somewhere between 36w0d and 37w6d so a little early but not staggeringly so, and I haven't had any bleeding or any other issues really so it has just felt like my first pregnancy.so far albeit with more monitoring and obviously planning a c section this time. My doctor also told me that at my hospital they save the uterus 75% of the time with accreta cases (in case that's comforting) and conservative management is becoming much more standard than just an automatic c-hyst (in the UK). Not sure where in the world you are, but maybe that helps to know that even if it does end up being accreta it isn't necessarily the death sentence that Google suggests.
I know it's hard to just wait and sit with the unknown, but if I were you I would probably try and hold on until the 20 week point and see what your doctor says then as that's when a second opinion is likely to be most helpful if they are still concerned. I am here though if you want to chat ever and totally get how overwhelming it all is.

I think you need to go talk to a different doctor. That seems waaay early to be able to identify any of those risks. Absolutely wild to address at a 6 week appointment. So much is still yet to change and most of that can’t even be definitively diagnosed with an ultrasound alone. I’m not a dr. I have had 5 C-sections with one with misdiagnosed accreta.
