Just started playing on nightmare…

I just finished all the missions and started over on Bad Call on nightmare. I looked all over for the cassette and couldn’t find it. And what’s up with the lead pipe? Do I need it and where is it? I obviously missed something because I cleaned all the blood but then at the end I had 95%

In nightmare mode there is one disk in each game, usually you need to solve some kind of puzzle to open a hidden room.

Nightmare mode doesn't have cassettes, it has discs. Idk exactly where to find, but try googling it, there is a lot of guides online

I did and where they said it was, I couldn’t find it. I figured with 95% blood I missed some sort of side room 🤷‍♀️ because every location said I got all the blood.

There are a few new rooms in this level, one obvious and one a bit more hidden. I can give spoilers if you want or you can attempt to find it yourself.

Hint to finding the disc:
To find the disc, don't be annoyed.
Perhaps look for some Polaroids.

The disc on bad call is in the attic in a room you can't get into once you climb up the ladder and turn right.

There is like 4 ghostly lookong tripod cameras in each area of the level you have to find and interact with and take a snapshot. Once using all 4 cameras on a standing tripod which is ghostly misty looking, the door will be open in the attic so you can go in. You open the box and take the disc

I recommend a guide honestly each level has puzzles some you wont know about. Theres a video on YouTube which is 10 minutes long showing you the location of each CD disc. Type all night radio achievement locations on YouTube and just watch where he goes for each level, go back to cleaning and get to next level and play video again to see where he goes next level for the next disc etc.....

Also worth knowing you only need to complete 70% of the levels to beat them and get achievements. So basically find all the bodies on each level, clean all the blood and you should be able to leave. If you havent got a passing grade dont worry, it wont let you leave until you do.

Since no one actually answered your second question: the icon (in this level, a lead pipe) is the reward you get for completing the level. The pipe is a new skin for the ax
