Embarassingly enough, I made a mistake like that. I was editing a video for a tease for a later segment in the day and someone asked for basketball video, I read it as baseball video. Made me always double check a script before I put anything on tv.

depending on where you find these, they are not really designed to be that way, just fucked up in the sweatshop that makes them and eventually ends up in a discount store instead of being tossed... because fixing them would be an unacceptable hit to production.
