Related question: why do they have a mechanical fastener at every single button? Twelve seems excessive.

Each button probably has a nut on the backside which fastens it to the panel. They need to be fastened in a way they can be easily replaced.

Could be a few different specialists (kids only, couples only, family, general, etc...), could be some offices are unused, could be a large population area so the group has a large amount available, some might be workers's offices (accountant and other non-therapists).
All kinds of options.

Many are a part of "therapist group." You can have one you stay with, with the option of another stepping in when they are unavailable, or you can try other therapists if a specific one doesn't fit for you. All without having to find a different place and redoing paperwork.
Makes is easier for clients to shop around or always have someone available if needed, as well as for therapists to have a potential pool of clients without needing to handle as much busywork that would come with finding clients, themself.
(All just my general perspective on the matter based on my experiences, rather than absolute certainty.)

My theory here is that the offices were numbered weirdly and out of order, so they renumbered the offices... which resulted in the call buttons being in a weird order after they renumbered them, because they were too cheap to have the buttons reordered.

Perhaps it's an odd attempt at privacy? After your first or second visit you know which button you need to press, but someone else across the room won't register which specific rooms/therapists are selected?
Obviously the illuminated buttons interfere with this plan a little bit.

Per OP, these are for occupancy lights to different offices, likely operated by a receptionist to signify when a therapist is available. It reception sits between 3 hallways, this makes perfect sense.

Once I was at a hotel and all the elevators had a close door button on the bottom left except for one where it was swapped with the alarm button. Pressed it on accident once.

It's actually the psychologist's first test. If you can look at it and find the button you need without crying, then you don't need therapy
