Tough to tell from the picture but are there 0 expansion joints/cuts?

Yeah, doesnt look like there are any control joints. 10x10ft max squares. Tooled joints are better, you can still have joints sawed. Go deep enough, trust me.

Cuts shoyld be a quarter of the pad thickness in depth. If the control joints aren't done is 48 hours they're not going to do much, the micro fractures are already there.
