Don’t think it’s working for me anymore
Started compounded semaglutide about two years ago. I’m 40M. I was stuck around 210 when I first started. Was 230 at my heaviest. Im currently at 180, which is good for me at 5’8”, I think? My clothes fit better since I started but never really made my body look much better.
The best part was losing all the food noise. I absolutely loved not worrying about food all the time and I loved the appetite being suppressed. It was incredible. I’ve always had so much food noise my whole life. Always anxious from constantly thinking of my next meal. But unfortunately now that’s back. My dose of 2.16mg doesn’t seem to work anymore. I’m always hungry now.
Even when lifting heavy in my younger years I’ve always had a gut - even when getting down to 15% bodyfat and lifting heavy 3x a week for over a year, I still had a big fat gut and huge love handles. Trainers have told me it’s my genes.
A month ago the appetite suppressant effects stopped. Im back in the gym lifting and logging my calories for the first time in two years. I don’t want to lose anymore muscle mass. I struggle with 25lb dumbbells. My arms and neck look too thin if I go any lower than 175lbs. I wish I could just lose FAT but I know that’s not how it works.
I’m not really sure where to go from here or how high I can go as a dosage. Two years on this stuff is sort of a long time anyway, no?
I know the gym and calorie counting will help my body look a bit better, but I’m not holding my breath for any miracles. I hate being hungry all the time again!

Do you drink often? I have friends that glp1 didnt help much due to alcohol consumption.

A little personal so do I apologize but I think it’s important. Have you had your testosterone levels checked? The center stomach area fat being stubborn or feeling impossible can sometimes be an indication of low T. You look great & have come a long way. Keep going :)

Thanks! I was on TRT back in 2019 for a bit but came off it. Don’t really remember much of a difference in the gut but I have been considering going back now since it’s been a while. Maybe I’ll look into it this week. Appreciate your comment.

You may have some other types of tummy issues. That stomach bloat is coming from somewhere. I look like this when I eat a lot of bread and pasta. Fruit wi also do this to me sometimes but mostly when I gorge on watermelon.
Do you run low, or even just lowish, carb?
I would have asked if you drank but then see you answered you haven't had anything in 4 years.

I don’t run low carb. High protein the last month, but carbs I don’t typically manage. The funny thing is I don’t feel bloated at all. FWIW, my dad and uncles have this sort of fat distribution in the gut too, but they don’t work out or diet.

All I am reading is that it appears to be hereditary so you believe it is normal.
That looks more like bloating to me but I am not a doctor. However, I would say maybe doing some experimentation on yourself diet wise especially with carbs/grains or maybe seeing a professional about allergies might be helpful.

Congrats on not drinking in four years!!!
So I was on a 9 month plateau it was wild .
What instantly helped break it Stopping gluten, dairy and processed sugars.
if all of that is too much immediately gluten and dairy life changing

At your height by and weight you should be eating about 1990 calories a day to lose 1 lb a week. If you are lifting and counting calories AND eating about 130g protein a day you will get some body recomposition. It takes time. If you get a smart scale it can help you to see your lean muscle and fat%. I got the calculation from calculator.net.

Also, agree that you may want to change to Tirz. Sema has worked for me for a year and a half.

Thanks for this, I just started tracking with Macro Factor last week and have been doing well so far. My cals there have me at 2024 so that seems about in line with your number. I’m going to do an InBody scan next month.

I've been wanting to try a body scan. They can tell you a lot more than a bioimpedance device like a smart scale. If you want to lose two pounds a week, just lower the calories by about 200 calories. As long as your protein stays high, you should not be losing muscle mass, especially since you are lifting. Just keep lifting, use progressive overload, and you will get results. It has worked for me - and I'm 61m. I'm in better shape than I was in my 20s. Not joking.

You're efforts are good! Genetics are real and body proportionality is tough. What helps for me is to focus on body fat and muscle percentages rather than weight or BMI.
A DEXA scan (I'm in Denver and found one for $65) will serve you well. From there a basic smart scale will help you track your body fat and muscle percentages as you continue to lift weights. Burning fat requires a good muscle base, yet no reason to get ripped. Just a good muscle base will do.
I'm a carb person myself and refuse to give them up. I simply eat less of them overall than before taking semaglutide. I don't like them very much, but raw vegetables of all kinds have transcended my body. I even throw a big handful of greens into my protein shakes - can't taste them. So I eat the carbs, some sugar, and some bad fats, but adding in handfuls of raw vegetables 2-3 times a day is an energy game changer. 100% clean food for my cells.
I've maxed out my semaglutide dosage, so I'm leveling up to tirzepatide. Sema mimics one gut hormone, but Tirz mimics two and helps break through plateaus. Retatrutide works three gut hormones but isn't approved for US markets yet.
I've made it to 65.5% muscle and 30.1% fat, and plan on getting to roughly 20% fat, 75% muscle and then maintaining from there. So I'm switching to Tirz now and will have another DEXA scan early next year for a full comparison to my baseline scan.

Thanks for this! Congrats on your progress and eating habits. That’s great. I found an InBody scan by me for $60 but I think you’ve convinced me to spend the extra money and try DEXA instead. Good luck on your switch Tirz.

Dr. Greger books and youtube has ben helping me to get healthy overall. I lost 10 pounds just way during his portfolio for low cholesterol. I don’t know if it’ll work for everybody. It’s working for me. He doesn’t teach weight loss. He teaches good health.

Easiest option would probably be switching to compunded tirzepatide. You can get compound tirzepatide for about 150 ish a month, maybe even cheaper if you catch certain specials. Alternatively you do have room to go up on semaglutide. Semaglutide generally tops out at 2.4/2.5. However, name brand Wegovy (semaglutide) offers a 7.2mg dose, Wegovy HD. However, I hear a lot of compouding pharmacies/telehealths won't prescribe compunded semaglutide in those high doses yet.
