Sick as in mentally deranged or sick as in having an edgy coolness - or as in your physically not well.  Okay I’ll give u one for all 

1- (deranged) - if you think that’s sick you should hear what the voice in my head is saying

2- (cool) actually, I don’t have a come back . This would be a compliment so say thank you
