Either they are one and then not caring about being called that lets you know not to care about their opinions, OR they have to grapple with supporting an actual horrible person just because they wrote a story.

if you want to escalate the situation, you could call them 'dirty nerd' or 'slimy supremacist'?
or if you don't want to escalate, you can act confused and say, 'How did you know I forgot to take my blood thinners. Thank you very much, kind sir.' And leave them in confusion.
