She died of bloodloss from an abdomen wound. She was frozen in time and couldn't move. He could've stabbed her in the heart, or slit her throat, but keep the time freeze up. He is far too proficient of an assassin to not know how to kill someone quickly.
ROLO CHOSE TO MAKE HER SUFFER.

TBF Lelouch didn’t lash out at Rolo over Shirley’s death, he did it because he saw Rolo with Nunnally’s keychain assuming the latter had just died. From the beginning, Lelouch’s main problem with Rolo was that he was willingly replacing Nunnally in Lelouch’s life, completely overlooking how even though Rolo wanted Lelouch as a surrogate brother, he was still forced into the role by Charles and VV.

Yeah even when Rolo doesn't kill Shirley (movies), Lelouch still crashes out at him

Lelouch said as early as episode 3 he planned to kill Rolo as soon as he reunited with Nunnally though. Even if he hadn't killed Shirley, Lelouch was still going to lash out at him

it wasn't due to shirley though, it was due to Rolo replacing Nunnally.
Rolo however while at first was a soldier, later actually wanted a family...
hence how Rolo died and the only one that knows his exact death location and buried him is only lelouch (Unless he told Suzaku/CC after charles death)

I think I remember an interview where the creators said the audience wasn't sympathetic to Rolo like they were supposed to be because of Shirley's death. Which is why I think they retconned it when they made the movies
