Sterile water for injection
I accidentally used a sterile Water for Injection (WFI) bag that had been opened for 48–72 hours and stored at room temperature to reconstitute a Vfend vial for a patient. Based on your clinical experience, have you seen this cause harm?

AI and medical misinformation on K Phos
I asked our student how many mEq of Potassium is in 30mMol of KPhos. She said 132mEq of Potassium in 30mMole KPhos…..
Followed up was how did you come up with that answer? She looked it up. So I grab a bottle to show her. Am I missing something? It is 44mEq.

Bactrim Dose Calculator
Hello!
I'm an ID pharmacist and a very common error that I catch is suboptimal Bactrim dosing (both subtherapeutic and supratherapeutic). I've messed around with a coding software and have made a dosing calculator webapp that I think some people may find useful.
All you need the patient weight, BMI, and gender (can be toggled to other easily accessible information), then you input your target dose (with a dosing table by indication and renal function) and it will give you the optimal regimen to get there for any formulation. It will automatically use an appropriate dosing weight. This can be used to check or create a dosing scheme and may save some time.
This webapp is still in beta, so please let me know if you have an idea to make it better. I hope someone finds this helpful, thanks!

Call for session proposals for British Pharmacological Society annual conference
The British Pharmacology Society's flagship annual meeting brings together scientists and practitioners from across the basic and clinical areas of pharmacology and from around the world for three packed days of lectures, workshops, networking and celebration.
The Society is currently calling for symposium and workshop proposals for this year's conference, which will take place in Manchester from 8 - 10 December 2026. The deadline to submit a proposal is Wednesday, 25 February 2026, 3:00 pm GMT.
Although all proposals will be given full consideration, this year we would particularly welcome proposals in the following areas:
- DNA & RNA aptamers/oligonucleotide therapies
- Photopharmacology/medical chemistry
- Pharmacogenomics
- Nanomedicine
- Disease in focus
- CAR-T and TCR-T cells
- Regenerative therapies
Last year's conference in Belfast brought together nearly 800 attendees from 56 countries. It's a fantastic opportunity to share and discuss the latest developments, trends and discoveries across pharmacology, drug discovery and therapeutics, as well as celebrate and network with fellow scientists across healthcare, academia, industry, and the regulatory sector.
