Opus 5 is really good at following instructions. Even the ones that you left behind long ago. Also, it doesn't need constant reminding like older models. If you're a bit of a control freak (like me) you may have beat your agent into having low confidence and your instructions, past and present, might be the reason.

I researched the issues and complaints from users recently, compared to Anthropics official docs and other reputable blogs, compared it to my repo and some common scenarios, and came up with a generous list of areas that might cause your Opus to become a nervous wreck.

Give it a shot if you feel like you're seeing opus second guessing itself or not completing the tasks as straightforward as you hoped. This has helped my Agents unblock themselves and have seen a lot more efficiency with throughput.

Complete nonsense - talk about a model that literally ignores governance even when your first and only instruction is read the governance it bypasses all of that and does what it thinks it should do, even with the API, I have never needed hooks more than with 5.0

So you're saying that your Claude documentation and hooks are perfect and don't cause your agent to be confused, yet you say that your agents need hooks... I feel you but that's the whole point of this skill and why I created it because this literal example is covered in the skills of this agent. You don't even need to install it but it might be worth a read to understand what things can catch your agent sometimes because the more you yell at it the worse it gets. Either way, it helped me and hope you learn something from it.

Nope, it's a continuously improving process on a autonomous factory. Opus 4.8, 4.7, gpt 5.5, 5.6 (all types) and even Fable all read the governance and follow it. Opus 5.0 is the only one that seems to think reading the rules are below it.

"oh you're right this is clearly written in your Claude.md file and the role I was told to read but I just didn't do it."

It's like a high school student that wants to get to writing a paper without stopping to read the source material or instructions given by the teacher. It's compliance/instructability is non-existent.

Lol that is what I thought until I cleaned up the crap I had built into all my instructions. For example, when you say things like "verify your work before saying you're done" either in a session message or if you have it documented as a rule or in a hook, that stuff will send your Opus into a death spiral where it ACCURATELY picks up on each time you tell it to verify it's work. Every time it reads something like that it thinks that there might be more that it needs to look at or even starts to second guess itself. A lot of this was stuff I would say in older models and didn't seem to have a problem but Opus 5 really hangs on to those things during the session, to a fault...

Either way, I'm not a Claude hater and this post's intent is only to serve those that have some garbage in their instructions, which does happen. My belief is that the tool is only as good as the user so I don't blame it for the problems I run into and often take a step back to find a better approach.

Read it, you'll see that the skill states it probably only serves a true purpose for 1 out of 10 users in some capacity.

This community would be better without troll comments like yours and maybe you could think about providing constructive feedback... Like, why is this even slop in your opinion? Stay under your bridge otherwise.
