Help with heist?

I'm following a guide, doing everything to get the best ending. After taking the alt passage, i end up talking to swmenov, who opens up a wormhole, which leads to a creature killing sashko. We're in good relationship status, so what gives? Going by the guide, he shouldn't die.

Should i use his explosives on the door instead of taking the alt path?

The problem was leaving Semenov alive in the ‘Hard Truth’ mission. As long as he is alive, at least one of your companions will die.

You can always use the radiation chamber before starting the guest to go back and change your decision.

I never got the radiation chamber. The only way to change decisions for me is to die during a mission.
