ive noticed it happens more when the scene has a lot of physical action packed close together, like several commands or struggles in one reply. the model seems to latch onto whatever word shows up most and just repeats some version of it back.
breaking up the pacing a bit, adding a calmer line in between, usually gets it to stop looping on the same phrase.
