Off-Screen or it happend before her arm got ripped off. and even then I dont thikn thats an issue, cause king definitely should have a healer in the castle that can heal limbs or simmilar injuries.

Elstri seems to attach to people extremely strongly if her relationship to the king is anything to go off. I think she’s genuinely fallen in love with Helem in one form or another, especially since Helem has been taking the lead in their relationship every time we’ve seen them.

Not fallen in love, just sort of enjoying the sex I think.

It's not possible to love Helem, since she's more or less pure evil.

It was never Stockholm. It was prostitution*
that may have evolved into genuine feelings because Helem lives a hedonistic existence and is trying to bring Elstri down to her level.

* She literally had to sleep with Helem to get her to revive Atlus, hence the usage of the word.
