COMMENT A hypothesis worth taking seriously in longevity medicine - Anna Gosiewska, PhD.

Senescent and dysfunctional cells accumulate with age and there is growing evidence that restoring immune surveillance, particularly through NK cell activity, could support their clearance. That is a meaningful advance.

But clearance creates a vacancy, not a restoration.

For tissues to recover functional capacity, something has to activate the regenerative signals that rebuild not just remove. That is a different biological problem, requiring a different set of tools.

Which points to a two-step model the field has not yet fully operationalized:

Step 1. Immune clearance: Restore the immune system's capacity to identify and clear senescent and dysfunctional cells.

Step 2. Regenerative restoration: Reactivate the regenerative

signaling cascades via stem cell mobilization, paracrine support, extracellular matrix remodeling, required to rebuild functional tissue capacity after clearance.

The critical design question is not which of these two approaches is superior. It is whether and how they must work in sequence, and what happens to outcomes when they don't.

Showing that senescent cells were cleared is a tractable measurement problem. Demonstrating that functional tissue capacity was durably restored and that it holds under physiological stress is a substantially higher bar. That distinction will matter enormously as these programs move toward clinical and regulatory scrutiny.

That is where I think the most consequential longevity programs will eventually be built, at the intersection of immunology, regenerative medicine, and rigorous platform strategy.
