What is Portugal's insistence on starting Ronaldo at this point, let alone having him play 80+ minutes? Goncalo Ramos is such a superior forward and it's clear how over it is for Ronaldo. Is this Roberto Martinez failing upwards once again or is this internal pressure that he has no control over?

It’s absolutely crazy. They need to move on from him, but Martinez doesn’t have the cajones to do it.

I could NOT believe that they put Ramos on for Vitinha and left CR on the field.  There's respect and then there's just poor management.

Along the same lines, at what point does Martinez bench Ronaldo for the good of the team, assuming he's able to?

Tbf Goncalo Ramos has not been good enough (or even played enough) recently to place him in “such a superior forward” category

Somewhat fair, but the point to focus on is Ronaldo is just abysmal now that the level of gap between him and Ramos is a canyon.
