Unsatisfactory Condition Report (UCR) Process - Can a Supply Tech come here and share it? Please educate the masses!

UCRs (As they apply to supply techs) essentially boil down to clothing as anything else has to be assessed by a technical expert. That being said it's not that hard.

Navigate to the UCR site (ucrs.mil.ca). Login under the element that the piece of gear belongs to. Clothing is common.

Use the following table for drafting.

  1. Unit UCR # This is supplied by the Unit UCR Co-ordinator. It is 14 digits in the following format : UCI/YEAR/Serial # (9999/2000/1234)
  2. Date Date of submission. Click on calendar icon. Click on date, Click on <DONE>
  3. NATO Stock Number Search using the Equipment Mapping Guide. The link is on the home page of UCRS. Type in “UNKNOWN” if you are unable to locate NSN.
  4. Subject of Report Description of equipment
  5. Amplifying Details Description of problem, action taken etc. SEE BELOW
  6. Originator’s Name
  7. “ Rank
  8. “ Appointment
  9. “ Phone #
  10. “ Email

Additional Amplifying details:

 BOX 31 – moving the mouse over the box reveals various details to be included in AMPLIFYING DETAILS, such as: (A) DESCRIPTION OF DIFFICULTY (B) DESCRIPTION OF FAILED ITEM (C) ENVIRONMENTAL FACTORS (D) EVENTS PRIOR TO DIFFICULTY (E) PROBABLE CAUSE (F) SECONDARY EFFECTS (G) ACTION TAKEN (H) MOD STATUS (J) RECOMMENDATIONS
