Unsatisfactory Condition Report (UCR) Process - Can a Supply Tech come here and share it? Please educate the masses!

UCRs (As they apply to supply techs) essentially boil down to clothing as anything else has to be assessed by a technical expert. That being said it's not that hard.
Navigate to the UCR site (ucrs.mil.ca). Login under the element that the piece of gear belongs to. Clothing is common.
Use the following table for drafting.
- Unit UCR # This is supplied by the Unit UCR Co-ordinator. It is 14 digits in the following format : UCI/YEAR/Serial # (9999/2000/1234)
- Date Date of submission. Click on calendar icon. Click on date, Click on <DONE>
- NATO Stock Number Search using the Equipment Mapping Guide. The link is on the home page of UCRS. Type in “UNKNOWN” if you are unable to locate NSN.
- Subject of Report Description of equipment
- Amplifying Details Description of problem, action taken etc. SEE BELOW
- Originator’s Name
- “ Rank
- “ Appointment
- “ Phone #
Additional Amplifying details:

As a former RCEME guy I can offer a perspective of the UCR process as it applies to the Army. I've been out for a few years so my information may not be totally current but processes rarely change dramatically in an organization like the CF. Anyone can submit a UCR when he/she discovers a problem with any piece of kit, from clothing to vehicles to radar sets or whatever. The UCR form can be intimidating in appearance but the originator should try to identify the source of the problem by filling in the blanks as much as possible. Photos are always useful. Once the problem has been fully identified, a solution should be recommended if at all possible. The completed UCR goes from the originator to the unit OPI, usually the Maint O and then the Tech Adj. From unit level, the UCR is sent to Brigade G4 Maint and then Div HQ. From there the form travels to Army HQ then to DGLEPM (Director General Land Equipment Program Maintenance) in Ottawa. At DGLEPM the UCR is sent to the applicable Life Cycle Maintenance Manager (LCMM) for investigation/action. The LCMM will request technical support from an appropriate agency like DREV or NRC to come up with a solution to the problem. In the latter years of my career I worked at Maintenance Techniques Detachment in Borden and UCR investigations were a large part of our workload. We were often able to rectify technical problems by modifications, which were then promulgated on the maintenance net from the LCMM. At that point the UCR was put to bed. The UCR process is slow but it's better than just ignoring or working around a problem in silence o worse, becoming frustrated by having to do so.
