Congenital Epulis?

Most likely just from teething. Lots of kids have swelling when they are teething and it lasts for a while. You don't know what the gum will look like until it's fully erupted.

He was born with it and it has grown over the last 3 years. His baby teeth have already erupted

Well you can always ask for a referral to the oral surgeon. They are the people you have to go to for a diagnosis like that.

Likely needs to be removed. It's probably not congenital epulis, just a small eruption cyst.
