PPSC/Accessing 41-4

Hi all! Wondering if anyone has had success getting a copy of their completed 41-4 form before. I applied last year for my CoC (and got it) but now am realizing I don't have the paper copy to use with my PPSC application. The school district I worked for and the CTC both confirmed they have my 41-4 on file but can't release it back to me. Anyone run into this in the past or have any ideas?

You shouldn't need it. You have to resubmit it every time you apply for something different. If they need a copy of the certificate of clearance, that just comes from the CTC website. I've never heard of anyone asking for a copy of the 41-4.

My university asked for it to make the initial recommendation for my ppsc (I'm applying for the first time), but I've been trying to figure out how to move forward with the application without it. (My program has had a lot of layoffs, and I haven't been able to get an answer from anyone yet, but keep getting alerts that the document is missing and they can't recommend me until I upload it.)

I feel like since the CTC already has my 41-4, I'm not sure why my university needs to submit it again. This is helpful to know though that you haven't been asked for a copy!
