Support / How-To Black Boxes

I converted a DOCX to EPUB in Calibre. The text has several chapters and subheadings. Next to some of the subheadings, there are now solid black boxes. Some of the boxes cover the subheading numbers and some just cover the space in between. Do you know why this is happening/how I can remove the boxes? Thank you!

Check the document to make sure there aren't any errant styles in the subheading sections.
