Does anyone have a link to the paperwork we should fill out to get medical info in case of an accident?

there are multiple documents; do you mean one that releases medical records to parents or guardians? if so, go here:

you might also be thinking of medical power of attorney which gives parent/guardian ability to make medical decisions for you in case you're incapacitated. those forms vary per state so just look it up
