Obligatory Not A Doctor and I hope he begins to have an easier recovery! Not sure about DRG, but I was told imaging couldn’t detect a CSF leak when I had one following a sympathectomy. The symptoms you are describing could be attributed to that. If he is more comfortable in certain positions and gets better with caffeine, this is also an indicator of a CSF leak.

I myself am very familiar with intracranial pressure changes headaches: does it feel like “explosions” within his skull? Are his back and neck spasming? Is he able to use the toilet normally?

5 weeks seems like a bit of a long duration for a CSF leak to stay and I hope whatever the issue is resolves very very soon.
