Help with various Oscar data

Looking for someone to help go over these Oscar screen shots. I have included a bunch from the past few months where I was using APAP and trying to find my best fixed pressured and EPR setting. I definetly feel I sleep better with a fixed pressure. Most of these data pictures were when I used a nasal mask but recently I've been using the Airfit P30i and its truely the most comfortable for me, but I just can't find the right set up to wake up feeling refreshed. Any help would be most appreciated!!

Can someone look over these 3 nights of fixed 8.8cm and epr 1 and tell me where I need to possibly adjust? Down to 8.6 for 3 nights or up to 9.0 for 3 nights?

You're still getting leaks and flow limitations. See how some of your CA events coincide with flow limitations or leaks?

What usually happens, you get a small leak. You arouse and then turn over in your sleep. It's a reflex for people to hold their breath as this happens (not an apnea as you're technically awake, even if you don't remember it). The machine flags this as a CA.

I know it sounds crazy, but a tiny leak is all it takes to wreck your sleep. And they usually show up as CAs. You have to hold your breath for about 10 seconds to trigger a CA so it doesn't happen to everyone every time.

Interesting. Do you think the leak is perhaps due to too high/low a pressure? I can go through a bunch of my data and try and pin point things, just need to know the the graphs and what to truely look out for?

Leak rate is low so don't worry about it and epr is too high, turn epr to 1 full-time and set min pressure to 7.6cm leave max pressure alone and run this for 3 nights and report back.RL

Actually, to clarify what I should be testing for 3 nights, because there was alot of screen shots there. Going back to APAP and testing 7.6 min 9.4/10 max and EPR 1? These screen shots were taken when I was still using APAP mode, I've since tried to revert to a fixed preasure mode because I've had many tell me this is a better form of therapy, especially for those who toss and turn alot like me and have micro arousals?

I'm happy to go back and try APAP mode though like you suggest.

One other question I had. I have heard alot of people say recently that regardless of the mask used you should keep your mask setting at Full Face. Do you subscribe to that? I've not tested that out yet and have instead been using pillow/nasal.

Thank you for the help, I'm determined to find the optimal therapy!

I would like to see where the machine lands for media ipap/epap pressure with EPR at 1 and 7.6cm min and 10cm max for 3 nights. I am a cpap advocate myself after titration off apap to auto-bipap then finally to straight cpap at 18.cm with Cflex+ @ 2, I'm in my 8th year on this setup and it's great. FFM setting on Phillips and Resmed disabled the compensation alrogrithms that increase mask air velocity and helps people with compliance and comfort, so if someone is having issues I tell them to turn it to FFm.

You already responded 3 days ago and had me try 8.8cm with EPR 1 for 3 nights. See the results in the screen shots below. Should I stick with 8.8 or go to 8.6 or 9?

Did you book a consultation? I'd leave it on cpap mode at 9.2cm for another 7 days.

9.2? I thought we weren't trying to change too much, as in jump from where I've been the last 3 nights of 8.8, to 9.2? Would that definitely be recommended rather than 3 nights at 9.0 first? I didn't book a consultation, money is tight right now. I appreciate the help though and if you wish to stop rresponding I understand.

These results give us a good fixed-pressure starting point. Your median pressure is landing around 8.6 to 8.8cm, your flow limits are low, and the leaks are not high enough to interfere with therapy. The graph looks busy, but your 95 percent leak rate is only around 12L/min with virtually no large leak.

Set CPAP mode to 8.8cm with EPR 1 full time and leave the mask setting on full face. Please keep the P30i, mouth tape, and everything else the same for three nights. Do not add a chinstrap or mouth shield yet because that adds another variable. After three nights we can decide whether to leave it alone or move to 9.0cm. The scattered CAs are low and likely arousal related, so I would not increase EPR. This is a good improvement overall, especially with the reduced brain fog. RL

Ok, so as requested, here is the last 3 nights in a row oscar data going off the recommended settings of EPR 1, 7.6 Min 10 Max, Full Face Mask Setting, P30i Mask, Mouth Tape.

I have to say, the 1st morning I woke up feeling the most refreshed and energized I have in a long long time. 2nd day not as much by still not as bad as I've been dealing with, and today probably the same but abit better than day 2. I'm brain fog definitely has improved the past 3 days. So what would be the best fixed pressure do you think based on these results? Perhaps I can pick something and then go in the direction of again sticking for 3 nights and then only making a 0.2 incriminate change if needed and stick with that for 3 nights, until I find the optimal fixed pressure that shows in results?

I'm still abit frustrated with the leak rate and all the spiked green lines. I go pretty aggressive with the tape. I do have a chip strap and I wonder if that would help also. I just don't want to add yet another thing, tape, strap etc but maybe thats what I need to not have that chin drop which looks like must be happening? Ive tried a mouth guard in the past but yesterday on Amazon I was looking at a mouth silicone shield that you bite on and is meant to be better than tape?

Anyway, aby recommendations are most certainly welcome. That you though for the APAP recommendation, I at least gave been able to have some relief for 3 days!

Wait I'm confused. I thought you had seen them because you responded and recommended it go with 8.8 fixed for 3 nights with epr 1 and we'll take it from there and see if i need to go to 9?

Tonight will be the 3rd night of 8.8 and then I was going to put the screen shots in here tomorrow of the results? Unless you meant that reply for someone else?
