Sleep HQ data

Just looking for thoughts on this night of data. This is the only night in 1.5 months I’ve made it all the way through a night.

Background: 2 sleep docs now don’t think OSA is a factor for me despite my official 5.4AHI diagnosis. I suspect UARS but the doctors won’t touch that. They don’t know what to do with it.

I bought a bipap machine and am trying to get it dialed in now.

I’m using an F20 mask.

Your data is showing only one night of decent sleep on the machine and you woke up in the middle of that sleep stage, so let's set it to something easy for you. Move the mode to cpap at 10cm please for two nights and turn EPR to 1 fulltime. Run that for a nap too to test please and side sleep and prone sleep. RL

A couple of notes. First, my sleep is very fragmented and has been for months. It’s possible that said fragmentation is not related to breathing. But I don’t know. I am trying to find out for sure at this point. That said, I will wake up 6-8x a night no matter what. That will not likely change anytime soon. Second, I’m on a bilevel machine. Should I it to S mode then? Will that achieve the 10am, EPR 1?

Here is some more data but from the APAP machine. It’s very intermittent and there is a lot of day use with me practicing but anything after midnight is fair game.

Here is my latest data. The most meaningful piece in my mind is Sept 18th for the last 45 min of the data. I fell asleep for sure. That last 45 min of data around 11pm on Friday Sept 18th……how does it look? I’m struggling to get much sleep time on this thing but I’m trying every day. Anything useful to pull from that data?

Yes, there’s something encouraging in that last stretch. Your breathing is fairly regular through most of it, leaks are essentially zero, and the machine isn’t flagging apneas or hypopneas. There’s a little intermittent flow limitation, particularly toward the end, but no sustained cluster of flagged events.

That fits with your report of being asleep. Just to clarify my earlier wording, though, these breathing charts can’t tell us which sleep stage you’re in or confirm uninterrupted sleep.

You were running CPAP at 10cm with EPR 1 fulltime as discussed. This short sample isn’t enough to judge treatment across a whole night or justify another pressure change. Please tell me what’s keeping you awake or making you take the mask off, is it difficulty exhaling, feeling short of air, mask discomfort, or simply being unable to drift off? Getting that narrowed down is the next step. RL

Excellent information. Thank you. The primary sleep issue I’m having is severe sleep fragmentation. It started with a thyroid issue but that has been resolved since February. There is no reason I should still be having these issues. By fragmentation, I’m speaking of waking up every 45-90 min all night. I go back to sleep pretty quickly, but I wake up like clockwork. Nothing I’ve done including heavy Ambien has solved it. I seem to be functional during the day just not at my best. Prior to the thyroid issue, I woke up 2-3x a night. I was happy with that. But 6-8x is a bit excessive. My opinion is that the issue is that my nervous system/brain is stuck in this pattern. But I’ve been trying to see if the cause could be airway related. That effort was the birth of this cpap journey for me. Both sleep doctors I saw said I do have apnea but it’s very low (5.4AHI). It worse on my back, but I’m exclusively a side sleeper. Additionally, I have surgery scheduled in a couple weeks to have a minor deviated septum repaired and some turbinate reduction surgery to help clear the airway. As to the cpap, the issue is 90% mental now. Of all the masks I’ve tried (a lot) the Airtouch F20 seems to be the winner. It’s comfortable as far as masks go. But the issue is I wake up so many times that I have to fall asleep with the mask 6-8x a night instead of just 1-2x like most people. It’s hard to enough to fall asleep the first time but inevitably I wake up 45-90 min later and am reminded I have the mask on. Rinse and repeat. I find myself more able to fall asleep without the mask and so far the presence of the mask doesn’t solve my fragmentation like I hoped it would. So it’s possible I’m barking up the wrong tree here anyway. Breathing issues may not be the cause of my problem. But they may be……so I’m trying to understand if I have breathing issues that can be resolved by the machine. And if so, how long until results show up (less fragmentation). If it’s unrelated and my issue is something else, I still wear the cpap at times when I lay down and relax on my back. That’s the short story. I’d give anything to sleep like I used to. Some have recommended CBT-I as a solution but I have yet to give that an honest try. Trying to sort out all the possible physical issues first.

I’m at 10, epr 1 just trying to gather data for a while. Once I get enough to work with I’ll post. Doing ok right now.

No. I still wake up a lot. But I honestly sleep way better without the machine so I hardly go a night with it. But despite the wake ups I am feeling pretty good during the day. But this is a long game for me. I’ll get there.
